6.- Signed legal representation form

C) THE HOLDER IS A DECEASED PERSON

1.- Technical sheet
2.- Circulation permit.
3.- DNI of the deceased.
4.- Death certificate.
5.- DNI of wife or children and family book
6.- Signed withdrawal form
